Zusammenfassung

Patienten mit coronarer Herzkrankheit erleiden postoperativ in etwa 7% und damit zehnmal sooft wie Patienten ohne coronare Herzkrankheit einen Myokardinfarkt (Tabelle 1), der in 50% tödlich verläuft (Tabelle 2). Aus klinisch statistischen Untersuchungen geht hervor, daß eine Reihe von anaesthesiologisch beeinflußbaren Parametern für die Häufung postoperativer Herzinfarkte verantwortlich zu machen ist. So ist bekannt, daß der Zeitraum zwischen einem praeoperativem Infarkt und dem Operationstermin eine Rolle spielt: Bei Operationen innerhalb der ersten 6 Monate nach dem Infarkt liegt die Frequenz der Reinfarkte bei 50% (Tabelle 3). Ferner ergab sich, daß die Hypertonie, die Anämie und der intraoperative Blutdruckabfall die postoperative Infarkthäufigkeit signifikant steigern (Tabelle 4).
