Abstract
Extracorporeal membrane-lung oxygenation (ECMO) has been largely used since 1971 for supporting life during severe respiratory failure [1, 10, 11, 13, 21]. Nevertheless, an overall survival rate not better than 10%-15% has led to increasing disappointment. Years of repeated evaluations of hemodynamics and gas exchange in patients with acute respiratory failure have yielded an improved understanding of the pathophysiology of this condition. Assessment of pulmonary function of patients on venoarterial bypass (VABP) gave essential information for the management of long-term extracorporeal circulation. This study, performed during ten cases of membrane-lung respiratory assistance, was undertaken to specify the relationship between the pulmonary circulation and gas exchange in severe acute respiratory failure patients treated with VABP.
