Zusammenfassung
Zur Behandlung der schweren HerzinsuffIzienz bzw. des „low output-“ Syndroms werden heute in zunehmenden Maße Substanzen mit positiv inotroper Wirkung eingesetzt. Die hierbei verwendeten Catecholamine unterscheiden sich vor allem durch ihre gleichzeitigen positiv chronotropen, arrhythmogenen und vasculären Nebeneffekte. Hierdurch werden die Einsatzmöglichkeiten dieser Substanzen bei Patienten mit kardialer Grunderkrankung häufig begrenzt. In dieser Situation ist die Entwicklung einer Substanz mit kardiospezifischer Wirkung von besonderem Interesse. Dobutamin, ein synthetisches Catecholamin, besitzt nach den experimentellen Untersuchungen vor allem von Robie et al. (9) sowie von Vatner et al. (13) und von Tuttle und Mills (12) überwiegend eine positiv inotrope Wirkung, während die chronotropen, arrhythmogenen und vasculären Effekte gering waren. Die bisherigen klinischen Untersuchungen an Patienten mit schwerer Herzinsuffizienz bzw. „low output-“ Syndrom schein en diese Ergebnisse für Dobutamin im wesentlichen zu bestätigen (1, 2, 3, 5, 7, 10). Hierbei besteht das Ziel der Therapie nicht so sehr in einer Anhebung des Blutdrucks als vielmehr in einer Steigerung des Herzminutenvolumens.
