Abstract

Regarding the radical excision of the cerebral arterovenous malformation (AVM) all textbooks advise isolating and clipping or ligating first the arterial trunks supplying the malformation before dissecting it, and to terminate the operation by coagulating or clipping the venous drainage. This strategy was recommended by DANDY (1) who reported a disastrous issue in one case of a primary ligation of the venous drainage. One must remember that at that time angiograms were not available, and the patient was under local anesthesia. Nevertheless, since then everyone has followed this apparently rational recommendation.
