Zusammenfassung
Bei allen intraokulären Operationen ist es erstrebenswert, zu Beginn des chirurgischen Eingriffs eine Verminderung des Augeninnendrucks (AID) zu erreichen, da bei hohem Druck im Moment des Einschnitts der Augeninnenhalt vorfallen kann. Außerdem wird dadurch ein Bruch der Arteriae ciliares posteriores mit nachfolgender austoßender Blutung begünstigt. Die konstante Kontrolle des AID ist deshalb eines der wichtigsten Ziele bei der Durchführung der Anaesthesie. Sämtliche Anaesthetica - die Muskelrelaxantien einbegriffen - können sowohl direkte als auch indirekte Auswirkungen auf den AID haben. Das d-Tubocurarin z. B. hat eine direkte Wirkung auf den AID und zwar durch Lähmung der Musculi bulbi (1, 2 ). Da aber in diesem Fall durch die allgemeine Muskellähmung eine Hypoventilation erzeugt wird, steigt auf Grund der dadurch entstehenden Hypoxie und Hyperkapnie der AID ( 3 ).
