Zusammenfassung
In Anbetracht der Tatsache, daß akute Traumaereignisse bereits frühzeitig zu meßbaren Funktionsbehinderungen des Antiatelektasefaktors der Lunge führen (2, 3, 22., 23, 44 ) und derartig geschädigte Patienten meist langdauernder Narkosen bedürfen, gewinnt der Einfluß potenter Inhalationsanaesthetica auf dieses für die Integrität des Alveolarverbundes essentielle System (12, 25, 35, 42, 43) grundlegende Bedeutung. Da im Verlaufe jeder Allgemeinnarkose und den sich daran anschließenden Zeitraum nahezu konstant (1, 4, 6, 15, 16, 20, 24, 29, 33, 36, 37, 49).eine Abnahme der Compliance (C) sowie der funktionellen Residualkapazität (FRK) verbunden mit einer Zunahme von Verschlußvolumen (closing volume) und alveo-arterieller Sauerstoffdifferenz (A-a DO2) beobachtet wird, lag es nahe, diese Veränderung auf eine spezifische Beeinflussung des Antiatelektasefaktors durch diese Stoffe zurückzuführen. Ob derartiges für Halothan (Fluothane, Halothan Hoechst) (8, 40), dem augenblicklich neben Lachgas wohl populärsten Inhalationsanaestheticum, zutrifft, war bereits, wenn auch mit den unterschiedlichsten Ergebnissen, Gegenstand zahlreicher Veröffentlichungen (18, 19, 32, 34, 39, 45, 48, 51, 53).
