Summary
The pathomechanism of ischemic brain swelling was studied in cats following 1 h of complete ischemia of the total brain. Brain water after ischemia increased from 75.9 to 78.3 ml/100 g, accompanied by an increase in intracranial pressure to 45 mm Hg and a displacement of the cortical surface by 270 µ. Extracellular space during ischemia decreased from 18.9 to 8.5 vol %, and expanded to more than 18% upon recirculation. Post-ischemic brain swelling was due to vasoparalysis and to the equilibration of osmotic and ionic gradients between extracellular fluid and blood. Upon reactivation of the energy-producing metabolism, brain swelling was reversible within 2–3 h.
