Abstract

Although superficial excision of corneal opacities—keratectomy—has been practised for centuries, actual transplantation of corneal tissue began only approximately 150 years ago (Table 1). The most active pioneering work took place during the first 30 years of this century. It was, however, only after the second world war that keratoplasty became a routine procedure for the ophthalmic surgeon (Table 1). The most important factors responsible for this development are the improvement in surgical technique under the operating microscope with ultrafine suture material and instruments; the advent of antibiotics for the prophylaxis and treatment of postoperative infection; the use of corticosteroids in the prophylaxis and treatment of immunologic graft rejection and the increasing knowledge concerning the fate of the donor tissue with recognition of the importance of the

endothelium for the ultimate transparency and optical success of the graft. The introduction of alloplastic corneal tissue substitutes or keratoprostheses, in cases where a keratoplasty might have a poor prognosis, should also be mentioned (Cardona et al., 1972).
