Zusammenfassung
Während thromboembolische Komplikationen bei Krebserkrankungen seit mehr als 100 Jahren bekannt sind (Trousseau, 1865), hat die gleichzeitige Manifestation disseminierter mikro- bzw. makrothrombotischer Komplikationen und einer hämorrhagischen Diathese bei Tumorpatienten erst in jüngster Zeit wachsendes Interesse gefunden. Selye (1966) hat im Hinblick auf die nach den Vorstellungen der klassischen Blutgerinnungslehre scheinbar widersprüchliche Koinzidenz von Mikro- und Makrothrombose und hämorrhagischer Diathese den Begriff des „thrombohämorrhagischen Phänomens“ in die Literatur eingeführt. Der Begriff besagt, daß die Ausbildung intravasaler Mikro-oder Makrothrombose durch Aktivierung der Gerinnung gleichzeitig zu einer mehr oder weniger ausgeprägten latenten oder manifesten hämorrhagischen Diathese führt. Die hämorrhagische Diathese resultiert bei diesem Syndrom aus einem Mangel an Thrombocyten und verschiedenen Gerinnungsfaktoren, die im Rahmen der lokal oder generalisiert eingeleiteten Gerinnungsaktivierung verbraucht werden Lasch et al. (1961) sprechen unter Berücksichtigung pathophysiologischer Aspekte eines aus gesteigertem Umsatz resultierenden Verbrauchs an Gerinnungsfaktoren von einer „Verbrauchskoagulopathie” als Ursache der hämorrhagischen Diathese. Pathoanatomisches Substrat der u. U. lokal initiierten generalisierten Hämostasestörung sind vor allem in der terminalen Strombahn der Organe auftretende multiloculäre fibrinreiche Mikrothromben, ein Befund, der zu dem Begriff der „disseminierten intravasalen Gerinnung“ (McKay, 1965) geführt hat.
