Zusammenfassung
Die Erforschung der Krebsgenese und die Entwicklung von Krebsthera- pien besitzen einen großen Stellenwert im öffentlichen Gesundheitswe-sen. Einer Prognose der American Cancer Society zufolge werden in diesem Jahr allein in den USA 520000 Menschen an Krebs sterben und 1130000 neue Fälle von Krebs diagnostiziert [1]. Aufgrund mangelhafter palliativer Initiative werden bei den meisten Patienten im fortgeschrittenen Stadium starke Schmerzen auftreten [6, 9, 21]. Das Programm der WHO mit den 3 Zielen der Prävention von Krebserkran- kungen bekannter Genese, der Verbesserung der Erkennung und der Heilung von Krebserkrankungen und der Linderung von Krebsschmer- zen lief im Jahre 1980 an [19]. Die dritte Komponente dieses Programms wurde 1981 mit einer Datenerhebung in 5 Ländern (Brasilien, Indien, Israel, Japan und Sri Lanka) fortgeführt, durch die sich bestätigte, daß Krebsschmerzen oft nur unzureichend gelindert werden. 1982 wurde in Mailand eine spezielle Konferenz einberufen, bei der die Durchführen- den der Untersuchung von 1981gemeinsam mit 6 Experten auf dem Gebiet der Schmerztherapie die „Richtlinien zur Linderung von Krebs- schmerzen“ — Anhang Nr. 1 des ursprünglichen WHO-Dokumentes — aufstellten. Urn die Erarbeitung der ersten Ausgabe der Linderung von Krebsschmerzen sicherzustellen, wurden von 1981 bis zur Veröffentlichung im Jahre 1986 Feldversuche durchgeführt und Konferenzen abge- halten.
