Zusammenfassung
Narbige und strikturierende Veränderungen an den Akren von Fingern und Zehen ziehen — je nach Ausprägung — eine Minderung der peripheren Durchblutung nach sich. Ist die Knochendurchblutung stärker beeinträchtigt, können sich Osteolysen z.B. der Processus unguiculares der Zehen und Endphalangen einstellen. Die auslösenden Ursachen sind zumeist chronisch-traumatischer Natur (s. unten). Akute traumatische Einwirkungen, z.B. durch Hitze, Frost oder elektrische Unfälle, sind vergleichsweise eher selten. Der Pathomechanismus von Akroosteolysen bei systemischen Erkrankungen mit peripheren Durchblutungsstörungen (z.B.progressive Sklerodermie, hypertrophische Osteoarthropathie; Tabelle 8.1) ist letztendlich derselbe wie bei nichtsystemischen Ursachen: wird beim permanenten Knochenumbau (Remodeling) den funktionellen Hauptakteuren, nämlich den Osteoklasten und Osteoblasten, die Ernährung abgeschnitten, so kommt es immer zu einer negativen Bilanz mit Schwund des Knochens.
