Die modernen Tracheostomieverfahren nach Ciaglia, Schachner-Griggs und Fantoni — ein Methodenvergleich

Since 1992, we have used three types of percutaneous tracheostomy procedures which have been described as alternative methods to conventional tracheostomy. Based on our experience of more than 600 percutaneous dilatational tracheostomies (Ciaglia 1985), on 65 percutaneous clamp tracheostomies (Schachner-Griggs 1989, 1990), and on 25 translaryngeal tracheostomies (Fantoni 1997) the advantages and disadvantages are now demonstrable. Percutaneous dilatational tracheostomy is the method of choice for adult and ventilated intensive care patients. Due to a lack of data on complications and contraindications, the clamp method has not yet become widespread, the same being true for translaryngeal tracheostomy. The three new procedures of tracheostomy are attractive but invasive, and their use therefore requires subtle knowledge of technique and method.

Zusammenfassung

Alternativ zum konventionellen Vorgehen sind für Intensivpatienten inzwischen drei neue minimalinvasive Tracheostomiemethoden beschrieben worden, die seit 1992 von uns angewandt worden sind. Aufgrund der eigenen Erfahrungen von über 600 Punktionstracheostomien nach Ciaglia (1985), 65 Dilatationstracheostomien nach Schachner-Griggs (1989, 1990) und 25 translaryngealen Tracheostomien nach Fantoni (1997) werden Vor- und Nachteile dieser Techniken deutlich. Demnach ist die Punktionstracheostomie derzeit Methode der Wahl für erwachsene, beatmete Intensivpatienten. Die Dilatationstracheostomie hat bisher nur eine geringe Verbreitung gefunden, Komplikationsraten und Kontraindikationen sind noch nicht klar definiert. Gleiches gilt für die translaryngeale Tracheostomie. Jedenfalls sind die drei neuen Methoden zur Tracheostomie attraktive aber invasive Verfahren, deren Technik und Methodik vom Anwender subtile Kenntnisse erfordern.
