Probleme bei der Behandlung einer Knochen-Echinokokkose am Beispiel einer pathologischen Fraktur des Femurs

While in Latin America, echinococcosis is found in up to 76 cases per 100000 of population a year, it rarely occurs in Germany. In Europe, infections are caused by two main types: E. granulosus and E. multilocularis that usually form hydatid cysts in the liver (>70%) and in the lungs (>25%). Bone manifestation is only seen in 1%–4% of all infections, and more than half of these relate to the vertebrae and the pelvis. Extremities are a rather unusual location for hydatid disease and cause many problems — for the surgeon as well as for the patient — in their struggle to prevent amputation of the limb. Contemporary treatment consists of pharmaceutical and surgical strategies which are reflected in the case presented of a patient with a pathologic fracture of the femur.

Zusammenfassung

Während die Echinokokkose in Lateinamerika in bis zu 76 Fällen pro 100000 Einwohnern und Jahr auftritt, stellt sie in Deutschland eine seltene Erkrankung dar. In Europa kommen E. granulosus und E. multilocularis als Haupterreger vor, die vor allem in der Leber (>70%) und der Lunge (>25%) Hydatid-Zysten bilden. Knochenbefall kommt bei 1–4% aller Infektionen vor, wobei mehr als die Hälfte dieser Manifestationen die Wirbelsäule und das Becken betreffen. Der Befall eines Röhrenknochens stellt klinisch eine Rarität dar und bringt im Kampf um den Extremitätenerhalt — sowohl für den Chirurgen als auch für den Patienten — eine Vielzahl von Problemen mit sich. Die derzeitige Therapie umfaßt sowohl medikamentöse als auch chirurgische Maßnahmen, die anhand des Beispiels einer pathologischen Fraktur des Femurs dargestellt werden.
