Summary
Positron emission tomography (18-FDG-PET) has been established in the differential diagnosis of pancreatic tumours. This procedure as preoperative diagnostic tool, however, has its shortcomings because it is hardly possible to identify exactly the underlying anatomical structures. To make this possible, it is necessary to overlay MRT or CT scans with corresponding PET databases. The high sensitivity and specificity of the PET to detect pancreatic carcinomas increases even more due to this superimposition. Extrapancreatic uptake in particular may be differentiated from pancreatic masses. Image fusion of PET and MRT or CT scans allows exact anatomical correlation of pathologic PET findings in patients with pancreatic carcinoma.
Zusammenfassung
Die Positronenemissionstomographie (PET) mit 18-FDG hat sich bei der Differentialdiagnose von unklaren Pankreastumoren bewährt. Das Verfahren ist jedoch im Rahmen der präoperativen Diagnostik dadurch beeinträchtigt, daß eine exakte Zuordnung der Befunde kaum möglich ist. Durch die Überlagerung von MRT- bzw. CTSchnittbildern mit korrespondierenden PET-Datensätzen wird eine Zuordnung der pathologischen Befunde zu den einzelnen Organsystemen ermöglicht. Die ohnehin hohe Sensitivität und Spezifität der PET in der Erkennung von Pankreaskarcinomen wird durch die Überlagerung weiter verbessert, insbesondere werden extrahepatische Mehranreicherungen als solche erkannt. Die Bildüberlagerung von PET und NMR bzw. CT bietet somit die Möglichkeit einer exakten anatomischen Zuordnung der pathologischen PET-Befunde, beim Pankreaskarcinom.
