Summary
Surgical oncology of carcinomas of the colon is also surgery of lymph drainage compartments, including radicular ligation of the vessels and tissue planes. According to tumor localisation and safety margins at the resection lines, standardized surgical procedures with the aim of R0 resection can be defined. R-classification is a strong independent prognostic factor regarding survival benefit. Adjuvant chemotherapy in curative resected colonic cancer UICC stage III should be recommended in general but modifications of this regimen must be investigated in further studies.
Zusammenfassung
Onkologische Chirurgie des Coloncarcinoms ist eine Chirurgie der Lymphabflußkompartemente mit zentraler Gefäßligatur und Beachtung der sog. Grenzmembranen. Unter Berücksichtigung der Tumorlokalisation und Einhaltung der onkologisch notwendigen Sicherheitsabstände lassen sich chirurgische Standardverfahren mit dem Ziel einer R0-Resektion definieren. Die R-Klassifikation ist ein entscheidender unabhängiger Prognosefaktor. Nach kurativer Resektion eines Coloncarcinoms im UICC-Stadium III ist generell eine adjuvante Chemotherapie anzuraten, wobei Modifikationen dieser Zusatztherapie weiter überprüft werden müssen.
