Zusammenfassung

Bei höchstens 5% der Patientinnen, die heute wegen eines epithelialen, invasiven Ovarialkarzinoms operiert werden, wurde der Ovarialtumor bei der symptomfreien Patientin durch gynäkologische Untersuchung oder Ultraschall entdeckt. Alle anderen Patientinnen melden sich bis heute wegen ihrer Symptomatik. Im Rahmen einer Qualitätskontrollstudie deutscher Frauenkliniken fand sich 1994 bei den Frauen, die wegen eines Prozesses an den Adnexen operiert wurden, nur in 7% ein maligner Tumor. In etwa 70% handelte es sich um einen Prozeß, bei dem rückblickend kein operativer Eingriff indiziert war. Verglichen mit Daten aus den 60er Jahren (vor Einsatz der Ultraschalluntersuchungen) ist der Anteil dieser (unnötigen) Operationen von damals 21% auf jetzt etwa 70% angestiegen (Pfleiderer et al. 1976). Das bedeutet, daß einem Früherkennungsfall knapp 200 unnötige Operationen gegenüberstehen. Hier liegt heute die Problematik.
