Zusammenfassung
Motilitätsstörungen bestimmen den chirurgischen Alltag seit den Anfängen der Abdominalchirurgie. Sie sind als passagere Phänomene früh postoperativ sowie als persistierende chirurgisch induzierte, relevante Störungen dem Chirurgen geläufig. Die klinische Relevanz wurde in der Regel unterschätzt bzw. der pathophysiologische Hintergrund nicht weiter eruiert. Ursächlich hierfür war lange Zeit die fehlende Beschreibung normaler Motilitätsabläufe. In den letzten Jahren wurden eine Reihe physiologischer Motilitätsphänomene an Magen und Darm erfaßt und quantifiziert, so daß jetzt eine Darstellung pathophysiologischer Grundlagen relevanter Störungen möglich ist. Klinische Zustandsbeschreibungen lassen sich jetzt gestörten Motilitätsmustern zuschreiben bzw. sind einer chirurgischen Ursachenforschung zugänglich. In der Folge sollen daher gegliedert nach den einzelnen Organabschnitten des Gastrointestinaltraktes die chirurgisch relevanten Forschungsschwerpunkte sowie Ergebnisse und Entwicklungstendenzen dargestellt werden.
