Zusammenfassung

Charakteristisch für die akute Pankreatitis ist die große Variabilität des klinischen Bildes und im natürlichen Verlauf. Von besonderer klinischer Bedeutung sind die etiologischen Faktoren Alkohol und Gallensteinerkrankung sowie die hereditäre Pankreatitis. Nach klinischen Kriterien, insbesondere nach Art und Häufigkeit auftretender Komplikationen, werden zwei klinische Phasen im Verlauf der akuten Pankreatitis unterschieden [1]. Innerhalb der ersten 14 Tage kommt es infolge von Flüssigkeitssequestration in das Entzündungsgebiet und Freisetzung vasoaktiver und toxischer Substanzen zur Hypovolämie; es können Herz-Kreislauf-Komplikationen, insbesondere Rhythmusstörungen, Nieren- und Lungenfunktionsstörungen auftreten. Die Spätphase der akuten Pankreatitis wird von septischen Komplikationen bestimmt, insbesondere bei nekrotisierender Pankreatitis. Lokale und systemische septische Komplikationen bestimmen die Spätphase; Sepsis ist die häufigste Todesursache der schweren akuten Pankreatitis [2].
