Zusammenfassung

Gewichtsverlust ist eine der häufigsten Komplikationen im Rahmen der HIV-Infektion [65]. Er tritt oft als Frühmanifestation der Erkrankung auf und führt in vielen Fällen zu Kachexie. Die Kachexie ist Ursache für eine hohe Morbidität, unabhängig von der Immundefizienz, und beeinflußt die Lebensqualität der Betroffenen. Hinzu kommen Symptome wie Durchfall, Appetitlosigkeit, Schluckbeschwerden, Übelkeit, Erbrechen und das sog. Wasting-Syndrom. Das Wasting-Syndrom ist eine AIDS-definierende Entität, die durch kontinuierliche Gewichtsabnahme trotz ausreichender Nahrungszufuhr gekennzeichnet ist und nicht durch pathogene Erreger hervorgerufen wird. Möglicherweise ist das HI-Virus allein auf direktem oder indirektem Wege, der Auslöser des Wasting-Syndroms. Charakteristika des Wasting-Syndroms sind eine deutliche Verminderung der fettfreien Körpermasse („lean body mass“), des Speicherfettes und des Wasserbestandes des Organismus. Dazu kommen ein gesteigerter Ruhegrundumsatz („resting-energy expenditure“) und häufig erhöhte Zytokinspiegel von TNF-α1 IL-1 und IL-6, die ebenfalls zum Katabolismus beitragen [19, 24].
