Zusammenfassung
Im Anschluß an die HIV-Infektion tritt bei fast allen Infizierten nach unterschiedlich langer Latenzphase eine zunehmende Funktionseinschränkung und Deletion der CD4+-T-Lymphozyten (TH) ein. Dieser Funktionseinschränkung wird in der Pathogenese der HIV-Krankheit eine zentrale Rolle zugeschrieben. Die Ursache der Funktionseinschränkung blieb bisher ungeklärt. Diese Funktionseinschränkung der TH wird in der Regel als Anergie oder Toleranz interpretiert, ein Zustand, bei dem die TH stillgestellt werden und die Fähigkeit verlieren, Interleukin 2 (IL-2) zu bilden. Bei Nagern wird eine derartige Anergie durch bakterielle oder virale Superantigene (SAG) induziert. HIV und verwandte Viren scheinen in vivo auf TH eine den SAG ähnliche Wirkung auszuüben und eine Art „Selbsttoleranz“ zu induzieren. In neuerer Zeit wurde das Konzept der HlV-induzierten Anergie/Toleranz in Frage gestellt und statt dessen vorgeschlagen, daß HIV eine Differenzierung in Interleukin 4 (IL-4)produzierende TH2 oder in Interferon γ (IFN-γ-) produzierende TH1 induziert. Eine HIV-Infektion führt somit einmal zu einer Deletion der TH und zum anderen zu einer ausgeprägten funktionellen Störung der T-Lymphozyten. Wieweit diese beiden Phänomene miteinander verbunden sind, ist das Thema intensiver Forschung.
