Zusammenfassung
Die koronare Herzkrankheit (KHK) ist in den Industrieländern weltweit nach wie vor die Haupttodesursache. Sie kann sich auf verschiedenste Weise manifestieren, u.a. als klinisch manifester Myokardinfarkt (Herzattacke), als Angina pectoris oder Herzinsuffizienz. Die Behandlung dieser Erkrankungen wird zunehmend verbessert, und in den meisten Fällen kann den Patienten eine konservative oder operative Therapie angeboten werden, die voraussichtlich eine Kontrolle ihrer Symptome und eine günstige Beeinflussung ihrer Prognose bewirken kann. Es handelt sich jedoch um schwere Erkrankungen mit hoher Mortalität, und es sollten daher Präventivmaßnahmen ergriffen werden, um die Entwicklung einer Atherosklerose in den Koronararterien zu verlangsamen und die Frühmanifestation der Erkrankung bei den Patienten erfolgreich hinauszuzögern. Außerdem leiden viele Patienten mit ischämischer Herzkrankheit an Episoden einer stummen Ischämie, und bei anderen Patienten (etwa 20%) ist die erste, letzte und einzige Manifestation dieser zugrundeliegenden Herzerkrankung der plötzliche Herztod [7,15]. Bei solchen Patienten sind prophylaktische Maßnahmen der einzig wirksame Weg, die Prognose zu beeinflussen, es sei denn, im entscheidenden Augenblick steht zufällig sofortige ärztliche Hilfe zur Verfügung. Darüber hinaus ist der plötzliche Herztod so häufig, daß eine bedeutende Einflußnahme auf die koronare Mortalität durch ein Medikament nur dann möglich ist, wenn es das Risiko für den plötzlichen Herztod reduzieren kann.
