Zusammenfassung
Seit seiner Einführung im Jahr 1980 [1] haben zahlreiche Studien gezeigt, daß die Behandlung mit einem implantierbaren Kardioverterdefibrillator (ICD) den plötzlichen Herztod bei Patienten mit anhaltenden Kammerarrhythmien vermindert [2–9]. Die Senkung der plötzlichen Herztodrate auf ungefähr 1% pro Jahr durch die ICD-Therapie [2] hat sie zu einem wichtigen klinischen Werkzeug als eine Behandlung der zweiten Wahl bei Patienten gemacht, die entweder unter Antiarrhythmikabehandlung lebensbedrohliche Rhythmüsstorungen haben oder sich während des Austestens eines geeigneten Arzneimittels als refraktär gegen Antiarrhythmika erweisen. Des weiteren sind ICD zu einer Therapie der ersten Wahl für ausgewählte Patienten geworden, bei denen eine medikamentöse Therapie nicht evaluiert werden kann. Ob die ICD-Therapie für alle Patienten mit lebensbedrohlichen Rhythmüsstorungen zur Behandlung der ersten Wahl werden sollte, ist weiterhin unklar [10, 11, 12]. Ein starkes Argument zugunsten einer derartigen Strategic wäre der Nachweis einer Reduzierung nicht nur des plötzlichen Herztods, sondern auch der Gesamtmortalität. Sekundäre Endpunkte wie eine geringe ICD-bezogene Morbidität und eine bessere Lebensqualität würden eine solche Behandlung bei diesen Patienten ebenfalls unabhangig begünstigen. Die derzeit verfügbaren Daten reichen nicht aus, um diese Fragen zu klären, hauptsächlich aufgrund des Fehlens randomisierter Studien, in denen ICD mit konkurrierenden Therapien verglichen werden; darüber hin aus sind in den veröffentlichten Berichten unterschiedliche Defmitionen und Endpunkte verwendet worden, so daß ein Vergleich dieser Studien miteinander erschwert ist [11].
