Zusammenfassung

Die perkutane transluminale Koronarangioplastie (PTCA) gehört heute zu den etablierten Verfahren zur Therapie der koronaren Herzerkrankung. Ständig verbesserte Kathetertechnologie sowie eine gröbere Erfahrung der Operateure erlauben heute eine primäre Erfolgsrate von über 90%, obwohl zunehmend komplexe Stenosen diktiert werden [9]. Die Erfolge der Methode werden akut durch Gefäßdissektionen mit konsekutivem Gefäßverschluß und langfristig durch eine Restenoserate von 30–50% limitiert. Auch alternative Techniken, die Gewebe des atheroskleotischen Plaques entfernen, wie die Atherektomie oder die Rotablation, haben die Restenoserate nicht wesentlich senken können [1,20]. Besonders die Wiedereröffnung von Gefäßverschlüssen ist mit einer hohen Reverschlußrate belastet.
