Zusammenfassung
Der therapeutische Einsatz von Vitamin D bei der Psoriasis vulgaris beruht auf einer Zufallsbeobachtung von Morimoto und Kumahara, die bei der Behandlung eines Osteoporosepatienten mit 1α-Hydroxy-Vitamin-D3 feststellten, daß die gleichzeitig bestehende Psoriasis unter dieser Therapie abheilte [19]. Dieser Einzelbeobachtung folgten mehrere Studien mit einer größeren Patientenzahl über den Erfolg der Einnahme von 1,25-Dihydroxy-Vitamin-D3 (Calcitriol) bei der Psoriasis vulgaris. Es konnte zwar in der Mehrzahl dieser Untersuchungen eine Besserung der psoriatischen Plaques beobachtet werden, allerdings in den meisten Fällen keine komplette Abheilung [20, 24]. Eine höhere Dosierung von Calcitriol war nicht möglich, da diese eine zu starke calcicotrope Wirkung hervorrief. Eine relativ starke calcicotrope Wirkung von Calcitriol war überraschenderweise auch bei lokaler Anwendung zu beobachten [24]. Dies führte zu dem Bestreben, Vitamin-D-Derivate zu entwickeln, die die gleiche antipsoriatische Wirkung aufweisen, allerdings wesentlich weniger calcicotrop wirken (Calcipotriol, Tacalcitol). Obwohl in den letzten Jahren gezeigt werden konnte, daß bei entsprechend strenger Überwachung die orale Verabreichung von Calcitriol bei der Psoriasis sicher und daher auch vertretbar ist [21], wird die Zukunft allerdings der lokalen Anwendung von Vitamin D bei der Psoriasis gehüren. Es gibt jedoch Untersuchungen, die zeigen, daß das therapeutische Ansprechen auf Vitamin D vom Genotyp des Vitamin-D-Rezeptors abhängt. So konnten Holick et al. beobachten, daß Patienten mit dem Genotyp bb wesentlich besser auf Vitamin D ansprechen als Patienten mit dem heterozygoten Genotyp Bb oder mit dem homozygoten Genotyp BB [6]. Diese Untersuchungen lassen die Spekulation zu, ob nicht der Genotyp des Vitamin-D-Rezeptors ein guter Prädiktionsparameter ist, ob ein Patient auf eine orale Vitamin-D-Therapie ansprechen wird oder nicht.
