Zusammenfassung
Molekulare Marker der intravasalen Aktivierung von Gerinnung und Fibrinolyse sind bei vielen Krankheiten des Erwachsenen untersucht worden, so daß heute ein gut gerundeter Überblick besteht. Im Kindesalter sind solche Untersuchungen nicht sehr häufig erfolgt. Bekannt ist, daß am ersten Lebenstag auch bei gesunden Reifgeborenen eine proteolytische Situation besteht mit erhöhten Fibrinopeptid-A- und Bß-i5-42-Peptidspiegeln [15], mit gegenüber der Erwachsenennorm gesteigerten Konzentrationen an Thrombin-Antithrombin-III- und Thrombin-a2Makroglobulin-Komplexen, Prothrombinfragment 1,2 sowie D-Dimer [1,7,10], die dann rasch, noch innerhalb des ersten Lebenstages, weitgehend ausklingen [7]. Bei Neugeborenen mit Atemnotsyndrom steigen die Spiegel an Thrombin-Antithrombin-III-Komplexen weiter an oder bleiben hoch, während Thrombozyten, Antithrombin III und andere Komponenten stetig weiter abfallen [3,11,12]. Bei der Neugeborenensepsis werden noch höhere Markerwerte erreicht [9, 10], besonders bei Schockzuständen. Über das Verhalten der Marker bei Erkrankungen jenseits der Perinatalperiode sind noch weniger Informationen verfügbar. Das Anliegen dieser Arbeit bestand darin, das Ausmaß der intravasalen Bildung von Thrombin und Faktor Xa sowie der Aktivierung der Fibrinolyse bei kranken Kindern aller Altersstufen zu ergründen, wobei septische Zustände besonders berücksichtigt worden sind. Zu prüfen war, ob diese Marker diagnostische Wertigkeit für das Grundleiden besitzen. Dann sollte ermittelt werden, welchen Markern die beste Sensitivität zukommt und ob eine kombinierte Bestimmung von Markern sinnvoll ist.
