Zusammenfassung
Es gibt nur eine sehr kleine Gruppe von Patienten mit hohem Leidensdruck, bei denen die Indikation zu einer chirurgischen Maßnahme unter der Diagnose „Dyspepsie“ überhaupt diskutiert werden sollte. Dies sollten die Patienten sein, die über viele Jahre letztlich erfolglos konservativ behandelt wurden, bei denen sich durch wiederholte Untersuchungen pathologische Motilitätsphänome im Magen nachweisen ließen und bei denen möglicherweise auch Schleimhautschäden der Magenmukosa makroskopisch oder histologisch auffallen, so z. B. bei therapierefraktäre Magenentleerungsstörung oder ein pathologischer duodenogastraler Reflux [4, 5, 21]. In diesen Fällen funktioniert der physiologische Ablauf der antroduodenalen Fortbewegung nicht, und im Falle einer totalen Entleerungsstörung kann durch eine distale Magenresektion der Übertritt von Nahrung in den Dünndarm ermöglicht werden, oder ein duodenogastraler Reflux kann durch eine duodenale Umleitungsoperation beseitigt werden [5, 7, 8].
