Surgical Anatomy of the Biliary Ducts

Abstract

Diagnostic and therapeutic procedures for biliary tract diseases have been improving; many surgeons have used new equipment in endoscopic biliary surgery and employed aggressive surgical approaches to difficult biliary cancer such as hilar cholangiocar- cinoma, with varying Degrees of success (Tio et al. 1991; VErbeek et al. 1992; NImura et al. 1988,1989; NIMURA 1993). Laparoscopic cholecystectomy has brought not only various advantages but also, unfortunately, complications in the form of proximal biliary stricture (CArroll et al. 1994; BRanum et al. 1993). Detailed anatomical studies of the hepatic ducts including the hepatic hilus have contributed new ideas and approaches for further technical developments in hepatobiliary surgery to treat difficult proximal biliary complications and hilar cholangio-carcinoma (HEaley and SChroy 1953; HEaley 1954; COuinaud 1954, 1957, 1989; BIsmuth 1982; BLumgart et al. 1984; MIzumoto et al. 1986; SAkaguchi and NAkamura 1986; NImura et al. 1990). This chapter describes recent anatomical studies which are relevant to the surgical treatment of difficult biliary lesions at the hepatic hilus.
