Zusammenfassung
Das Zervixkarzinom entsteht überwiegend im Bereich des Übergangs vom unverhornten Plattenepithel der Portiooberfläche zum drüsenbildenden Zylinderepithel des Zervikalkanals. Wie das carcinoma in situ ist auch eine geringer ausgeprägte Epitheldysplasie eine obligate Präkanzerose, beide werden nach CIN Grad I-III (Cervikale intraepitheliale Neoplasie) klassifiziert. Durch den zur Früherkennung eingesetzten zytologischen Abstrich (Cramer, 1974) steigt die Inzidenz der präkanzerösen Epitheldysplasie gerade auch jüngerer Frauen zunehmend an (Rummel et al., 1979). Als Standardtherapie der schweren Epitheldysplasie bzw. des carcinoma in situ wird die Konisation durchgeführt. Die histologische Aufarbeitung erbringt dabei entweder die erfolgreiche Exzision des atypischen Epithelbezirkes im Gesunden — eine engmaschige zytologische Verlaufskontrolle ist jedoch weiterhin nötig — oder macht eine weitergehende — aufgrund der hohen Rezidivrate dieser prämalignen Veränderungen oft auch mehrmalige - chirurgische Intervention nötig. Neben den physischen Folgen wie Zervixinsuffizienz oder Ausbildung narbiger Stenosen können dadurch auch die psychischen Folgen dieser Eingriffe erheblich sein.
