Zusammenfassung
Trotz großer Fortschritte in der intensivmedizinischen Behandlung schwerkranker Patienten stellen Sepsis und SIRS nach wie vor die Hauptursachen für die Entwicklung multipler, oft letaler Organfunktionsschäden im postoperativen/posttraumatischen Krankheitsgeschehen dar [1]. Klinische Studien zur Wirkung anti-inflammatorischer Monotherapien (Antikörper gegen Endotoxin und Zytokine, Zytokinrezeptorblockaden etc.) haben sich als nicht ausreichend multivalent erwiesen um das komplizierte Netzwerk interagierender Entzündungsfaktoren effektiv zu beeinflussen [4]. Erfolgversprechende neue Therapiestrategien müssen deshalb notwendigerweise auf Zielfaktoren mit multifaktorieller Mediatorfunktion ausgerichtet sein. Ob und inwieweit Antithrombin, ein bereits seit mehr als 15 Jahren in der Klinik verabreichter Hemmstoff des Gerinnungssystems diesen Anforderungen genügen könnte, soll im folgenden erörtert werden.
