Indikationen und Kontraindikationen der Nasennebenhöhlenchirurgie im Kindesalter

Zusammenfassung

Jahrzehntelang war die Behandlung der Erkrankungen des Nasennebenhöhlensystems (NNH) im Kindesalter eine Domäne der Pädiatrie und des konservativ tätigen Oto-Rhino-Laryngologen. Der HNO-Chirurg wurde nur dann hinzugezogen, wenn orbitale und endokranielle Komplikationen drohten oder eine Tumorkrankheit vermutet wurde. Dieses Konzept war einerseits Folge der unzureichend bekannten Physiologie der Nebenhöhlen und andererseits Ausdruck der berechtigten Furcht vor Komplikationen und späteren Wachstumsstörungen des Gesichtsschädels als Folge der osteoklastischen Operationsverfahren. Mittlerweile ist jedoch eine ausgesprochen gegenteilige Entwicklung zu beobachten: Ebenso wie beim Erwachsenen tritt auch beim Kind die chirurgische Behandlung der NNH zunehmend in den Vordergrund, und im klinischen Alltag kann man sich gelegentlich des Eindrucks nicht erwehren, daß sie immer häufiger als Therapie der ersten Wahl angesehen wird.
