Zusammenfassung
Bisher ist die lokale weite Exzision immer noch die Standardtherapie des Melanoms der Haut, obwohl sich die Sicherheitsabstände von ursprünglich dogmatisch vertretenen 5 cm Sicherheitsabstand auf nunmehr 10 und 20 mm sich verringert haben. Daher ist es nicht mehr weit zu den Empfehlungen im amerikanischen Schrifttum die mikrographische Chirurgie für die lokale Melanomthe-rapie anzuwenden. Die Durchführung dieser Methode beruht auf einer lückenlosen dreidimensionalen Darstellung der Schnittränder des Tumorexzisates und reduziertem Sicherheitsabstand. Gut bewährt hat sich die Methode bei der Lentigo maligna und beim Lentigo maligna Melanom und hat sogar schon Eingang in die offiziellen Leitlinien der Deutschen Krebsgesellschaft gefunden [6] Voraussetzung dafür ist eine kontinuierliche Ausbreitung des Tumors.
