Zusammenfassung
Die Endophthalmitis ist die gefürchtetste Komplikation nach einer Kataraktoperation. Zur Vorbeugung fügen die meisten Operateure der intraoperativ benötigten Spüllösung ein Antibiotikum hinzu. Es ist jedoch fraglich, inwieweit diese prophylaktische Maßnahme das Auftreten einer intraoperativen Kontamination zu verhindern vermag.
Nach dem Zufallsprinzip wurden bei der einen Hälfte unserer Kataraktpatienten 40 mg Gentamicin in die Spüllösung (500 ml BSS-Steril) hinzugegeben und bei der anderen auf diese Maßnahme verzichtet. Insgesamt wurden 699 Patienten in der Studie berücksichtigt. Bei allen wurde eine Phakoemulsifikation mit Implantation einer PMMA-IOL über einen 6,2 mm breiten nahtlosen korneoskleralen Tunnelschnitt durchgeführt. Präoperativ erfolgte eine intensive mehrmalige Wischdesinfektion der Bindehaut, Lider und periorbitalen Hautregion mit 11°/oiger PVP-Jodlösung. Am Ende der Operation wurden ein Punktat aus der Vorderkammer sowie eine Probe direkt aus der Spülflüssigkeit entnommen und mikrobiologisch untersucht.
Es fanden sich keine signifikanten Unterschiede hinsichtlich der Kontamination der abgenommenen Vorderkammerpunktate (p=0,57) und Spüllösungsproben (p=0,55) zwischen beiden Patientenkollektiven. Im Nachbeobachtungszeitraum von mindestens 3 Monaten trat in keiner der beiden Gruppen eine Endophthalmitis auf.
Vor dem Hintergrund einer möglichen Sensibilisierung und Resistenzentwicklung gegenüber pathogenen Keimen erscheint die weit verbreitete Praxis einer routinemäßigen intraoperativen Antibiotikagabe nach den Ergebnissen unserer Studie zumindest fragwürdig. Um eine endgültige Aussage hinsichtlich dieser Problematik treffen zu können, sind jedoch wegen der geringen Inzidenz einer Endophthalmitis nach Kataraktoperation weitere Untersuchungen an einem größeren Patientenkollektiv erforderlich. Eine entsprechende multizentrische Studie wird daher von unserer Klinik derzeit initiiert.
Summary
Postoperative endophthalmitis is the most serious complication after cataract surgery. For prophylaxis of endophthalmitis, most Ophthalmie surgeons add antibiotic agents to the intraocular irrigation solutions. It is questionable, how far these prophylactic procedures are able to reduce the risk of intraoperative microbial contamination of the solutions.
Six hundred and ninety nine consecutive patients scheduled for cataract surgery were randomized to receive irrigation Solution (balanced sah Solution) without antibiotic additive, or with 40 mg Gentamicin. In all patients, phacoemulsification was performed via a self-sealing 6.2-mm scleral tunnel incision with implantation of a PMMA-IOL. Preoperatively, all patients had a thorough swab desinfection of the periorbital skin, lids, and conjunctiva with an 11% PVP-iodine Solution. At the end of surgery, speeimens of anterior Chamber fluid and directly from the infusion line were withdrawn and processed for microbial culture. There were no significant differences in the number of microbially contaminated specimen either between the two groups, in the probes of anterior Chamber fluid (p=0.57), or in those withdrawn directly from the infusion line (p=0.55). Düring a postoperative follow-up period of at least 3 months, there was no case of postoperative endophthalmitis in either group.
Considering the possible sensibilization of patients and increasing microbial resistance to antibiotic agents, the widespread use of prophylactic intraoperative antibiotics seems to be at least questionable according to the results of our study. Because of the very low incidence of postoperative endophthalmitis after cataract surgery, a much larger number of cases needs to be studied to obtain reliable results. For this reason we are now initiating a multicenter trial.
