Zusammenfassung
In dem Übersichtsreferat wird zunächst anhand der Literatur auf die Häufigkeit einer Endophthalmitis nach IOL-Implantation eingegangen. Diese liegt bei 0,1-0,2%. Anschließend werden die Erregerquellen und die präoperative Infektionsprophylaxe besprochen. Unverzichtbar ist ein steriles Folienabdecken inklusive der Lidkanten mit den Zilien sowie eine gründliche Antisepsis unmittelbar vor Op-Beginn z. B. mit PVP-Jod. Die klinische Diagnose einer Endophthalmitis stützt sich auf die subjektiven und objektiven Symptome — abhängig von den Erregern und deren Lokalisation — und sollte mikrobiologisch durch Vorderkammer- bzw. Glaskörperpunktion gesichert werden. Die Indikation zur chirurgischen Therapie richtet sich nach dem klinischen Befund, sollte aber nicht zu streng gestellt werden. Häufig ist es günstig, mit dem Eingriff nicht nur die Erreger mit ihren Toxinen, sondern auch die körpereigenen reaktiven und ebenfalls schädlichen Produkte zu eliminieren. Bei der Gelegenheit kann dann das notwendige Antiinfektivum direkt appliziert werden, parallel zur übrigen topischen und systemischen Gabe.

Endophthalmitis after intraocular lens implantation. This survey Covers firstly the incidence of postoperative endophthalmitis (0.1–0.2%). Secondly, prophylaxis will be discussed. The lids and lashes should be completely covered and disinfection of the conjunctiva, i.e., with PVP iodine should be carried out. The clinical diagnosis of an endophthalmitis is based on several subjective and objective symptoms and should be verified by a puncture of the anterior Chamber or, better, of the vitreous cavity if infiltrated. Treatment with antibiotics/antimycotics and steroids is recommended. Finally, surgical therapy (ppV) will be discussed.
