Intraokularlinsen-Implantation bei Herpeserkrankung des Auges

Zusammenfassung

Intraokularlinsen-(IOL-)Implantationen in Herpesaugen erfolgen häufig sehr zurückhaltend, da mit vermehrten postoperativen Komplikationen gerechnet wird. Zur Klärung, ob diese Zurückhaltung gerechtfertigt ist, analysierten wir die Verläufe nach 112 perforierenden Keratoplastiken (Kp) in Herpesaugen (65 Kp ohne, 47 Kp mit IOL-Implantation) im Hinblick auf das klare Transplantatüberleben, die Inzidenz von Immunreaktionen und Herpesrezidive. Die Auswertung erfolgte nach Kaplan und Meier, auf statistische Signifikanz wurde mittels Log-Rank-Test geprüft. Sechs Jahre postoperativ waren ohne IOL 82,3%, mit IOL 85,0% der Transplantate klar. Ohne IOL waren 58,0%/71,8%, mit IOL 76,0%/70,2% der Transplantate frei von Immunreaktionen bzw. Herpesrezidiven. Keiner der Unterschiede war statistisch signifikant. Die Intraokularlinsenimplantation in Herpesaugen kann daher bei entsprechend sachkundiger Nachsorge als eine sichere und empfehlenswerte Methode einer funktionellen binokularen Rehabilitation angesehen werden.

Summary

Many surgeons refuse to implant intraocular lenses (IOL) into herpes eyes because various postoperative complications are expected. In order to find out if this attitude is justified, we analyzed the clinical courses after 112 penetrating keratoplasties (kp) in herpes eyes (65 without and 47 with IOL implantation) concerning clear graft survival, incidence of immune reactions and herpes recurrences. For Statistical analysis we used the Kaplan and Meier estimation and Statistical significance was evaluated via the log-rank-test. Six years postoperatively, without IOL 82.3%, with IOL 85.0% of the grafts were clear. Without IOL 58.0%/71.8%, with IOL 76.0%/70.2% of the grafts experienced no immune reactions/herpes recurrences. None of the differences was statistically significant. With Provision of thorough follow-up, intraocular lens implantation into herpes eyes can therefore be regarded as a safe and recommendable method of functional binocular rehabilitation.
