Zusammenfassung
Nach Einführung von Chlorpromazin als das erste Antipsychotikum Anfang der 50er Jahre hat die experimentelle Forschung sich sehr intensiv mit der Frage nach dem Wirkungsmechanismus der antipsychotischen Wirkung auseinandergesetzt. Nach vielen Rückschlägen sind wir heute in der Lage, eine relativ klares Bild zu formulieren. Im Zentrum der antipsychotischen Wirkung stehen antagonistische Eigenschaften an Dopamin-D2-Rezeptoren als der minimale gemeinsame Nenner aller heute klinisch eingesetzten Antipsychotika. Die wesentlichen Evidenzen für diese Aussage sind von mir erst kürzlich ausführlich zusammengefaßt worden (Müller 1998). Ein wesentliches Argument für diese Kernaussage sind vor allen Dingen auch Substanzen aus der Reihe der Benzamide (z.B. Sulpirid und Amisulprid), die pharmakologisch gesehen recht selektive D2-Antagonisten darstellen und sich in klinischen Studien als gut wirksame und anderen Substanzen gleichwertige Antipsychotika gezeigt haben. Während man im Falle des Chlor-promazins die zusätzliche antagonistische Beeinflussung einer ganzen Reihe anderer Rezeptorsysteme über viele Jahre primär als Ursache zentraler und peripherer unerwünschter Arzneimittelwirkungen angesehen hat, weiß man heute, daß eine zusätzliche Beeinflussung verschiedener Rezeptorsysteme neben den D2-antagonistischen Effekten (z.B. Dl5 D3> D4, 5-HT2) wahrscheinlich die Ursachen für die sogenannten atypischen Eigenschaften verschiedener modernerer Neuroleptika darstellen (Müller 1998).
