Zusammenfassung
β-Adrenergika leiten sich von Adrenalin ab, wobei durch die Modifikation der Sei-tenketten eine höhere β2-Selektivität und längere Wirkdauer erreicht werden kann. Vorläufer der selektiveren β2-Adrenergika sind Isoprenalin und Orciprenalin. Sie haben bei der Behandlung einer obstruktiven Ventilationsstörung keine Bedeutung mehr. Adrenalin wird unter besonderen Umständen und unter bestimmten Voraussetzungen in Einzelfällen subkutan oder langsam intravenös appliziert. Dies gilt v.a. für den Status asthmaticus. Man versucht dabei, die a-Rezeptoren der Gefäße im Bronchialsystem zu stimulieren, um durch gefäßabschwellende Wirkung die Obstruktion zu beeinflussen. Auch auf der Intensivstation wird Adrenalin manchmal eingesetzt, um durch direkte Instillation einen direkten Gefäßeffekt zu erzielen. Die arrhythmogene und drucksteigernde Wirkung des Adrenalins muß aber mitberücksichtigt werden.
