Zusammenfassung
Die Thoraxchirurgie beinhaltet die Operationen an der Lunge, der Thoraxwand und am Mediastinum, wobei die Herzchirurgie und die Chirurgie der großen Gefäße ausgeklammert werden. Zu den typischen Besonderheiten gehören die präoperativen Abklärungen, die speziellen pathophysiologischen Veränderungen und technischen Anforderungen im Rahmen von Ein-Lungenventilation und (Seiten-)Lagerung sowie das perioperativ oft kombinierte Auftreten von hämodynamischen und respiratorischen Schwierigkeiten. Entsprechend wichtig sind die intra- und postoperativ einzusetzenden Überwachungsverfahren. Neuere Entwicklungen wie thorakoskopische Operationen, die Lungenvolumen-Reduktions-Chirurgie bei Patienten mit schwerem Lungenemphysem und die uni- oder bilaterale Lungentransplantation stellen besondere Ansprüche an den Anästhesisten. Das erfolgreiche Lösen dieser komplexen Aufgaben setzt umfassende Kenntnisse der kardiopulmonalen Pathophysiologie und der spezifischen Pharmakologie voraus. Der Anästhesist muß zudem über besondere apparative und technische Fertigkeiten verfügen in den Bereichen Atemwegsmanagement inklusive Fiberbronchoskopie, Beatmungsstrategien inklusive Jetventilation, Schmerzbehandlung, vor allem thorakale Epiduralanalgesie, und Anwendung sowie Interpretation der hochtechnisierten Monitoringverfahren, wenn möglich inklusive transösophagealer Echokardiographie. Für besondere Fälle wird auch die Erfahrung mit dem Einsatz der extrakorporalen Zirkulation verlangt. Als unabdingbare Voraussetzung für erfolgreiche, komplikationsarme Thoraxeingriffe ist ein einvernehmliches, interdisziplinäres Vorgehen von größter Bedeutung.
