Zusammenfassung
Bei keiner anderen Arzneimittelgruppe wird das therapeutische Prinzip der Dosis-Wirkungs-Beziehung so häufig vernachlässigt wie bei den Blutprodukten, insbesondere bei der Gabe von gefrorenem Frischplasma (GFP; engl. „fresh frozen plasma“, FFP). Dies belegen viele Publikationen aus den verschiedensten Ländern, wo anhand von im Konsens festgelegten Therapieprinzipien die Substitution mit Thrombozytenkonzentraten (TK) in 20–40%, die Transfusionen von Erythrozytenkonzentraten (EK) in 30–50% und die Gabe von FFP in bis zu 80% der untersuchten Behandlungsfälle als inadäquat bewertet wurden. Die Kritik dieser Studien bezieht sich vorwiegend auf die Laborwerte, die vor der Therapieentscheidung als Maßgabe zur Transfusion in den Patientenakten vermerkt wurden [1, 17, 30].
