Zusammenfassung
Die klinischen Symptome einer intrakraniellen Drucksteigerung sind zumeist unspezifisch und äußern sich in Kopfschmerzen, Übelkeit, Schwindel, Erbrechen, Geh- und Sehstörungen und Somnolenz bis hin zur Bewusstlosigkeit. Gelegentlich wird in traumatologischen oder intensivmedizinischen Zentren der behandelnde Arzt mit der Frage konfrontiert, ob eine abdominelle Intervention laparoskopisch oder konventionell bei einem Patienten durchgeführt werden sollte, der ein Schädel-Hirn-Trauma erlitten hat oder an einer anderen neurologischen Erkrankung mit erhöhtem intrakraniellen Druck leidet. Die Indikation zum laparoskopischen Verfahren wird gegenwärtig noch sehr zurückhaltend gestellt, weil in experimentellen und klinischen Studien bei laparoskopischen Operationen mit einem Pneumoperitoneum eine Erhöhung des intrakraniellen Druckes (ICP) nachgewiesen wurde. Vor der Indikationsstellung zum laparoskopischen Eingriff muss deshalb zwischen den bekannten Vorteilen der Laparoskopie und den möglichen Risiken bei Patienten mit Schädel-Hirn-Verletzungen abgewogen werden, zumal eine Beziehung zwischen einem erhöhten ICP und einem schlechten neurologischen Ergebnis besteht.
