Zusammenfassung
Bereits seit längerer Zeit ist bekannt, dass ein erhöhter IAP durch Aszites, Ileus oder Blutung zu einer Beeinträchtigung der Nierenfunktion führen kann. Bereits 1876 untersuchte Wendt in Selbstversuchen den Einfluss eines erhöhten IAP auf die Urinproduktion[21]. Richards et al.[19], Kron et al.[14] und Jacques und Lee[10] berichteten von Patienten, bei denen nach massiver intraperitonealer Blutung mit erhöhtem IAP ein akutes Nierenversagen auftrat, das nach der Dekompression des Abdomens sofort reversibel war. Auch Celoria et al.[4] bemerkten eine Oligurie, die auf den erhöhten IAP durch einen großen Ovarialtumor zurückgeführt wurde. Von Harman et al.[6] wurde in einem Tiermodell nachgewiesen, dass ein erhöhter IAP von 20 und 40 mm Hg die Nierendurchblutung und glomeruläre Filtration vermindern. Obgleich die meisten laparoskopischen Operationen heute bei einem niedrigeren IAP durchgeführt werden, könnte ein Pneumoperitoneum möglicherweise die Nierenfunktion einschränken.
