Zusammenfassung
Auch bei laparoskopischen Operationen können größere Gefäße verletzt werden, die mit einem hohen Blutverlust einhergehen. Selbst wenn es gelingen sollte, eine kreislaufwirksame Blutung laparoskopisch zu stillen, ist es dennoch fraglich, ob das laparoskopische Vorgehen mit einem Kapnoperitoneum ohne Gefährdung des Patienten fortgesetzt werden sollte. Auch bei einem Polytrauma oder stumpfen Bauchtrauma könnte im Rahmen der weiteren Diagnostik eine Laparoskopie erwogen werden, sofern sich der Patient in einem stabilen Zustand befindet. Generell wird man sich darüber Gedanken machen müssen, ob Patienten mit den Zeichen einer aktiven oder stattgehabten Blutung laparoskopiert werden sollten. Möglicherweise schadet der negative Einfluss des Pneumoperitoneums auf das Herz-Kreislaufsystem (s. Kapitel 2) mehr als die laparoskopische Methode nutzt.
