Zusammenfassung

Das umfassende Assessment ist das Herzstück der geriatrischen Vorgehensweise. Sowohl die ganzheitliche Betrachtung des Patienten und Evaluierung der Gesundheitsprobleme auf physischer, psychischer und sozialer Ebene, als auch die Einbindung des therapeutischen Teams in Diagnostik und Behandlung sind im geriatrischen Assessment verwirklicht. Es ist der Schlüssel zu einer individuell sorgfältig geplanten medizinischen, pflegerischen und therapeutischen Behandlung mit dem Ziel, größtmögliche Selbständigkeit zur Bewältigung des Alltags wiederzuerlangen.
