Zusammenfassung
Mit dem Verfügbarwerden von Nelfinavir im Frühjahr 1997 eröffneten sich neue Möglichkeiten einer hochaktiven antiretroviralen Therapie von HIV-Patienten. Nelfinavir ist ein Proteaseinhibitor (PI) mit potenter Wirkung und günstigem Nebenwirkungsprofil [7]. Auch hinsichtlich der Entwicklung von Kreuzresistenzen zu Nelfinavir bei mit Protea- seinhibitoren vorbehandelten Patienten liegen günstige Daten vor [6, 8]. Stavudin (d4T) und Didanosin (ddl) sind seit mehreren Jahren verfügbare Reverse Transkriptase-Inhibitoren, die ein synergistisches Wirkprofil zeigen [3,9]. Durch die Einmalgabe von ddl sollte die bekannte Nebenwirkung der Pankreatitis reduziert werden. In dieser Studie werden antiretroviral vorbehandelte Patienten mit dem oben genannten Therapieregime über nunmehr 18 Monate behandelt und mit naiven Patienten verglichen. Dementsprechend ist es Ziel unserer 1997 begonnenen Studie, die Effektivität, Verträglichkeit und Sicherheit einer Kombinationstherapie mit Stavudin (d4T) und Didanosin (ddl) zusammen mit Nelfinavir im Langzeitverlauf zu untersuchen.
