Zusammenfassung
Antiretrovirale Therapie ist der rationale Ansatz, durch effektive Suppression der HI- Virusreplikation die Krankheitsprogression im Verlauf der HIV-Infektion zu verlangsamen. Dieser Ansatz war bereits Mitte der 80er Jahre zunächst in Monotherapien klinisch bei symptomatischen Patienten erfolgreich. Nach der Entwicklung neuer antiretroviraler Substanzen wurde zunächst Anfang der 90er Jahre belegt, daß die antiretrovirale Kombinationstherapie der Monotherapie überlegen ist. Wirksamere Kombinationen kennzeichnen seit Mitte der 90er Jahre die ≫hochaktive antiretrovirale Therapie≪ (HAART) und zugleich eine Abkehr von Studien mit klinischen Endpunkten hin zu virologischen und immunologischen Surrogatmarkern [5, 11]. Die Indikation zur antiretroviralen Therapie wird inzwischen früher gestellt und seit Einführung der HAART sind die HIV-bedingte Morbidität und Mortalität zurückgegangen [9, 12]. Daher sind klinisch definierte Endpunkte wegen der notwendigen langen Beobachtungszeiträume schwieriger zu realisieren. Unterschiedliche antiretrovirale Kombinationen haben sich als wirksam erwiesen, und ein prospektiver Vergleich der in aktuellen Konsensusempfehlungen [5] angegebenen verschiedenen Möglichkeiten wirksamer Kombinationen steht bisher aus.
