Zusammenfassung
Das akute Koronarsyndrom wird in der Mehrzahl der Fälle durch Ruptur einer atherosklerotischen Koronarplaque ausgelöst. Auf den exponierten thrombogenen Oberflächen der Gefäßwand adhärieren Thrombozyten. Experimentelle und klinische Studien haben gezeigt, daß der akute Myokardinfarkt durch einen Thrombus ausgelöst wird, der aus einem thrombozytenreichen Kopf und einem fibrinreichen Schwanz besteht und das Koronargefäß komplett verschließt. Die andere Form des akuten Koronarsyndroms, die instabile Angina pectoris, wird hingegen durch einen thrombozytenreichen, das Koronargefäß subtotal verschließenden Thrombus hervorgerufen. In beiden Fällen kommt somit den Thrombozyten pathogenetische Bedeutung zu.
