Ist die offene Gangrevision bei Choledocholithiasis noch indiziert?

Summary

In a German-wide survey (859 hospitals), 75% of the hospitals carry out therapeutic splitting with preoperative endoscopic revision (ERC), followed by laparoscopic cholecystectomy (LC) in cases of preoperative diagnosis of choledocholithiasis. Only 12% of the hospitals prefer the primary open cholecystectomy with choledochotomy. In cases of intraoperative findings of common bile duct stones, 16% convert to open surgery. Fifty-eight percent of the hospitals prefer postoperative extraction via ERC, or in 1.4%, the laparoscopic revision. In our own patients (48 with choledocholithiasis), the extraction of common bile duct stones with ERC was feasible in 97% of cases. Complications ranged around 2%. Two patients were treated by laparoscopic transcystic revision. There was no open choledochotomy necessary.

Zusammenfassung

In der deutschlandweiten Umfrage (859 Kliniken) führten 75% der Einrichtungen bei einer präoperativen Diagnose einer Choledocholithiasis die ERC und Extraktion vor geplanter laparoskopischer Cholecystektomie (LC) durch. Lediglich 12% der Krankenhäuser favorisieren die primär konventionelle Cholecystektomie (KC) mit simultaner Gangrevision. Im Fall einer intraoperativen Diagnosestellung bei einer LC, konvertieren nur noch 16%. 58% bevorzugen die postoperativ endoskopische Sanierung bzw. 1,4% die laparoskopische Gangrevision. Im eigenen Krankengut (48 Patienten) konnte eine Sanierung der Gallenwege in 97% per ERC und EPT erzielt werden. Die Komplikationsrate lag bei 2%. 2 Patienten wurden einer erfolgreichen laparoskopischen Gallengangsrevision unterzogen. Eine offene Choledochotomie war nicht notwendig.
