Die ossäre Metastasierung des Mammakarzinoms: Therapie und Prognose

Summary

Between 1980 and 1998,88 patients with bone metastases due to breast carcinoma were surgically treated. Fifty-six cases were seen at the extremity, 32 at the spine. Beneath several decompressing and stabilising procedures at the spine in the extremity group, a resection was performed in 11%, standard prosthetic replacement in 15%, tumor prosthetic replacement in 25% and compound osteosynthesis in 25%. Survival rates of 1,2 and 5 years were 75%, 45% and 22% respectively. Mean survival was 29 months in the extremity and 17 months in the spine group (ns). Location at the spine vs extremities, and age (≤6o vs >6o years), showed no prognostic significance. Solitary vs multiple bone lesions, as further visceral spread and duration of symptoms (≤3 vs >3 months) proved to be significant. The extent of surgery should reflect these findings.

Zusammenfassung

Zwischen 1980 und 1988 wurden 88 Patienten mit Knochenmetastasen operiert. In 56 Fällen lag eine Extremitäten-, in 32 eine Wirbelsäulen-Beteiligung vor. Neben dekomprimierenden und stabilisierenden Wirbelsäuleneingriffen wurde an den Extremitäten eine Resektion in 11%, eine Standard-TEP in 15%, eine Tumoren-TEP in 25% und die Verbundosteosynthese in 25% eingesetzt. Das 1-, 2- und 5- Jahres-Überleben lag bei 75%, 45% und 22%. Die mittlere Überlebenszeit betrug 29 Mon. in der Extremitäten- und 17 in der Wirbelsäulen-Gruppe (ns). Wirbelsäulen- und Extremitätenläsionen sowie Alter (≤60 vs >60 J.) zeigten sich nicht prognostisch signifikant. Singuläre versus multiple ossäre Beteiligung sowie die weitere Organbeteiligung und Anamnesedauer (≤3 vs >3 Monate) waren prognostisch signifikant. Die Radikalität des Eingriffes sollte entsprechend angepaßt werden.
