Tumorstenosen: Laser und Stents

Summary

Recanalisation of tumor stenoses is one of the most effective but most difficult interventions in flexible endoscopy. Analysing our own 5000 such interventions in the GIT between 1989 and 1999, indications, conctraindications,differential therapy and technical procedures are discussed, concerning special surgical aspects and multimodal therapies. Ablation (Nd: YAG-Laser, APC) and prostheses [plastic tubes, stents (SEMS)] have to be used not as alternatives but as components of an individual combination therapy, together with dilative methods. The “state of the art” and future aspects are discussed regarding innovative new technologies.

Zusammenfassung

Die Rekanalisation von Tumorstenosen gehört zu den effektivsten, aber auch schwierigsten Interventionen der flexiblen Endoskopie. Anhand des eigenen Krankengutes (> 5000 Rekanalisationen im GIT zwischen 1989 und 1999) werden die Indikationen, Kontraindikationen, die Differentialtherapie und die technische Durchführung, vor allem in Kontext mit chirurgischen Eingriffen und den neuen Modalitäten der neoadjuvanten und adjuvanten Therapie dargelegt. Ablative Verfahren (Nd: YAG-Laser, APC, Injektionsdestruktion etc.) und prothetische Verfahren (Plastiktuben, SEMS) sind meist nicht alternativ, sondern als Bausteine einer Kombinationstherapie zu sehen und zusammen mit dilatativen Methoden einzusetzen. Der derzeitige “Stand der Technik” sowie Zukunftsaspekte unter Berücksichtigung neuester Technologien werden dargestellt und diskutiert.
