Zusammenfassung
Bereits 1891, 30 Jahre vor der Entdeckung des Insulins, wurde von dem englischen Chirurgen Williams die Transplantation von Teilen eines Schafspankreas in die Subkutis eines komatösen Typ 1-Diabetikers durchgeführt (Williams 1894). Die erste erfolgreiche klinische Pankreastransplantation wurde durch Kelly und Lillehey 1966 durchgeführt. Es handelte sich hierbei um eine segmentale Pankreastransplantation in die Fossa iliaca. Das exokrine System wurde durch Ligatur des Pankreasganges blockiert. 1973 empfahlen Gliedman und Mitarbeiter den Harntrakt zur Drainage des Pankreassafts und Merkel berichtete über eine End-zu-Seit-Anastomose des Pankreasganges mit dem Ureter (Gliedman et al. 1973). Mitte der 70er Jahre führte die Stockholmer Transplantationsgruppe um Groth eine größere Serie von sog. darmdrainierten Pankreastransplantationen durch. 1978 empfahl Dubernard (Lyon) die Pankreasgangokklusion mittels eines Polymers (Dubernard et al. 1978). In dieser Technik wurde von Land 1979 auch in Deutschland die erste Bauchspeicheldrüsentransplantation durchgeführt. Bereits 1982 empfahlen Cook und Sollinger aus Madison eine Verbindung zur Harnblase und die erste klinische Transplantation wurde im Sinne einer Anastomose des Pankreasganges mit der Blasenmukosa durchgeführt (Sollinger et al. 1985). Sehr rasch entwickelte sich diese Technik in dem Sinne, dass das gesamte Pankreas mit einem Anteil des Duodenums im Sinne eines „Knopfes“, später mit einer gesamten Duodenaltransplantation und einer Seit-zu-Seit-Anastomose zwischen Duodenalsegment und Blase durchgeführt wurde. In den letzten Jahren unter dem Einfluss besserer Immunsuppression und größerer klinischer Erfahrung erlangte die Technik der Ableitung des exokrinen Anteils der Bauchspeicheldrüse in den Darm einen neuen Stellenwert. Sie wurde zum Standardvorgehen in den größeren Zentren.
