Zusammenfassung
Diffuse alveoläre Verdichtungen, welche auf das Vorliegen eines ▸ Lungenödems hinweisen können, sind ein häufiger radiologischer Befund auf Intensivstationen. Abgesehen von der Atelektase ist das Lungenödem sowohl die häufigste Ursache für einen Abfall der Sauerstoffsättigung, als auch für das plötzliche Auftreten von unerwarteten intrapulmonalen Verdichtungen im Lungenröntgen. Die drei prinzipiellen Mechanismen, die zur Entstehung des Lungenödems führen, sind: 1) erhöhter hydrostatischer Druck auf die Kapillarmembran; 2) verminderter onkotischer Druckgradient an der Kapillarmembran; 3) erhöhte Permeabilität der Kapillarmembran, die durch Schädigung der endothelialen Zellverbindungen zu einem Austritt von Flüssigkeit und Proteinen in den Alveolarraum führt.
